Congenital Pulmonary Arteriovenous fistula Treated with Therapeutic Embolization in an Adult

Abstract
Pulmonary arteriovenous fistula(PAVF) is a rare congenital condition originated from malformation of capillary development. Dyspnea, cyanosis, polycythemia may occur because of the direct entry of unoxygenated, desaturated arterial blood into the pulmonary venous system. It can occur either as an isolated process or in association with similar abnormalities of the skin, mucous membrane which is called as hereditary hemorrhagic telangiectasia (Osler-Weber- Rendu syndrome). Death often results from cerebral abscess and rupture of the malformation with massive hemorrhage. Therapeutic intervention is recommended for all symptomatic patients because of the risk of those serious complications. Recently we experienced a case of the pulmonary arteriovenous fistula which was confirmed by pulmonary angiography, Surgical lobectomy or pneumonectomy was the only treatment until the technique of embolotherapy by balloons or coils was developed. In order to preserve tissue in the left lung and to avoid a thoracotomy, we chose coil embolization. After embolization, the patient became acyanotic and symptom-free, Arterial PO2 and oxygen saturation has increased ignificantly and concomitant decrease in hemoglobin, hematocrit and RBC number has occurred.
